original medicare does have a premium for part b & part d, unless you're under the medicaid threshold in which case medicaid pays your medicare premium.

Ive lived in the Netherlands since the changeover from national healthcare to private "regulated" health care choice. The cost of premiums is a problem and the government isn't really sure about what to do. Its completely over rated internationally. (though still way better than the US system)
